She thinks she does not need it and so she is throwing it away.

Hannah begs you to keep this confidential and not tell anyone especially her daughter, who she sees regularly, as her daughter will be very angry. Bi How would you explain the term ‘confidentiality’ to Hannah? I would advise her that the term confidentiality does not actually mean that I can keep secrets for her and that I have to disclose information to my colleagues and sometimes external professionals. I would also have to keep records of any conversations we have . Keeping confidentiality means not discussing information with anyone outside of the social care setting

This could result in a lack of trust from Hannah towards me. Keeping this information to myself could result in me being disciplined and also could cause a serious health risk to Hannah.
